For patients who have filed or will file a Workers Compensation claim, the following is a list of documents you need to either review or complete for your first appointment. Links to these documents are available below and under the Medical Forms tab. Please complete these documents and bring them with you to your first appointment.

A list of your current medications
Completed office forms:
- Workers Compensation Information Sheet (please complete and sign)
- Authorization to Release WC Medical Information (please complete and sign)
